We rate practices, not people. Our 36-month window focuses on recent, verifiable behavior. Providers receive notice and a right of reply before publication or tier changes.
Evidence we accept (strongest → weakest)
- Primary documents: redacted evaluation letters, monitoring recommendations, SI correspondence.
- Corroborated testimonies: two or more independent reports describing the same practice.
- Public records: court filings, consent orders, published policies, on-record remarks.
- Expert statements: named SMEs with disclosed conflicts.
Submitters must verify identity to P4HR (kept private). We redact names/PHI and standardize submissions before review.
Scoring & Tiers
We compute a simple score from verified inputs. Positive practices can offset points.
- Each corroborated report = +1
- Each primary document = +2
- Each confirmed public record = +3
- Each positive practice = −1 (min 0)

Privacy & Redaction
- Remove names, MRNs, addresses, and any PHI before uploading.
- We may reject or delete submissions that include PHI or unredacted sensitive details.
