What the HIMS Program Is — And What It Has Become

The FAA’s Human Intervention Motivation Study (HIMS) program originated in the 1970s as a pathway to support airline pilots diagnosed with alcohol dependence and return them safely to flight. Over time, HIMS expanded far beyond its original scope and now functions as a de facto behavioral health monitoring framework influencing certification outcomes for a wide range of conditions and historical events.

In many cases, participation becomes a practical prerequisite for a pilot to obtain or retain a Special Issuance medical certificate. However, pilots report that the program operates without uniform, binding guardrails—creating an environment where monitoring can become prolonged, financially coercive, and susceptible to subjective decision-making.

Reform does not seek to eliminate clinically necessary monitoring. It seeks to modernize oversight so it is time-limited, evidence-based, independently reviewable, and consistent with constitutional safeguards.
